Medication at School

When it is determined that a student should receive medication at school, the following steps should be followed. The school nurse will be responsible for administration of all medication at Prairie Hill and Willowbrook Schools. Medication forms are available from the school nurse or by clicking this link.

The PHS Medication Advisory Committee has suggested that parents either deliver the medication to school themselves, or call to inform the school nurse that the medication is being delivered by the student.

Prescription and Over-the-Counter Medication

A written order from a physician or dentist is required for all medication. However, a current and correct prescription label on the medication container may be substituted for the written order.

Medication must be brought to school in the original pharmacy container.

A parent or guardian and the child's physician must sign the approved medication form  authorizing administration of the prescribed medicine at school.  If a child is to carry and Albuterol inhaler with them during school hours, a physician must sign the medication form stating this.

Non-Prescription Medication

To enhance student learning, increase student attendance, and reduce individual student's early dismissals, the use of non-prescription analgesics for symptom relief will be allowed with a physician's order. Only the school nurse will administer these medications.

Approved "as needed" medications are intended for occasional use only. 

To have occasional "as needed" medications provided to a student at school, complete the Prescription and Over-the-Counter Medication Authorization form.  Supply the medication in the orignial packaging with the student's name attached.

Written notification of non-prescription medication administration will be provided to the parent for each dose given. A parent may request telephone notification prior to each adminstration.
